Migrating from Exchange to Google Apps

Can anybody advise on migrating from Exchange Server to Google Apps, ie, the groundwork required, steps to be taken in order, likely problems, how to test before going live, in other words, everything.

What exchange server is being used? How many users? Do you want to integrate Google Apps with Exchange or do you want to completely retire the exchange server and use Google Apps
