"We are taking steps to see it never happens again". I think it is about time somebody came up with a new phrase, because we hear this old chestnut every time something happens such as unnecessary deaths in hospitals. Why the hell didn't they come up with the steps before the problem?

I'm not thinking of any one happening, but they seem to crop up regularly.

Purely on a matter of long-standing principle, I find it very difficult to utter the above phrase (except possibly on Xmas Day , well fed and glowing with good cheer!!) - yet in this case I must make an exception. We need to hear when things DO work.

You are perfectly correct about the media involvement - it's their salacious desire for profit - because everyday 'good things' don't sell papers.
