When to apply a default gateway?

If you are using a basic hub, nic cards and xp, cable modem ect, do default gateways have to be manually set?

Lets say I use two manually installed Ip's (192.168.1.10 & 192.168.1.20, set the subnet mask 255.255.255.0, how do I know which default gateway to use?

Do I select it manually eg - 192.168.1.1 or does the hardware specify it?

Just a bit confused on the gateway thing and how/when it's applied.
