What's my modem?

I have an irritating problem in that I can't figure out what modem I have and need drivers for it (to run XP Home).

On one side of the circuit board it says
80-203W230-2 and nothing else obvious on any of the chips.

On the other side we have the FCC No :
H8NTAI-35798-M5-E

and also this number on a sticker :
1456VQH3(INT)

I've tried these numbers in Google but haven't come up with anything positive. Does anyone have any ideas please?
