In Windows Explorer, right click a Word Document. Click the Browse button, select Microsoft Word and tick the box 'Always use this program...'.

Double-click My Computer then go to Tools -> Folder Options. Click the File Types tab then scroll down the list of extensions and click DOC to highlight it. Click the Advanced button and then there is a Change Icon button at the top. If you have to browse for an icon then on my XP setup the icons are in "C:\WINDOWS\Installer\{90110409-6000-11D3-8CFE-0150048383C9}\wordicon.exe" but they are also in "
C:\Program Files\Microsoft Office\Office11\Winword.exe".
