Lost heading on task manager

When using ctrl,alt, delete recently I inadvertantly erased the headings on task manager and now when I do ctrl,alt,delete and the "box" for task manager appears, it sometimes has up to 20 other boxes behind the original and on the lower task bar there are small green gridded squares, which show cpu and ?% running, if I right click it shows delete on the top and a tick against always on top in the lower panel.
I find the easiest way to delete the task managers is to right click the small green boxes as the close sign is no longer on the task manager.
Hope someone can help.
