I/O device error

I have a HP Media Center m7000kr Desktop PC Intel Core 2CPU [email protected]
1.58GHz,2.00 of RAM Windows XP

I have tried to insert my SD Card into the appropriate slot to print out photos, the message I am receiving

”Drive is not accessible, the request could not be preformed because of an I/O device error”

How can I fix this problem?

it could be that your card reader is not connected properly. More info please.

I have 1.5tb hard disk and partitioned it using windows vista, but after installing the windows in drive c: - the drive D: is not accesible and a dialog appears as I/O device error, any help possible from anyone?
