Is there any way to make windows always open up folders in full screen mode?

I'm not sure if this is what you want, but you can set Windows to always launch a program in maximised (full screen) view.

Right-click on the program's shortcut on the desktop, or in Start>All Programs. Then click on Properties, then the Shortcut tab. Click the down-arrow to open the Run list, then click on Maximised. Then click OK.

I often want to insert a file into an opened document. When I click insert and then the folder I want. it opens in a small window and at the top R H S is a ? and an X but nothing to maximise the window

If you are referring to the Open/Insert/Save etc dialogs in Microsoft Office then I don't think that it can be done. Not without some clever API programming anyway.
