email form creation in front page

i am creatng a feedback email form for my website. i have made quite good progress but io am now stuck. by default the form is saved to private/form.txt but i feel i need to change this, but i have no idea what to do. i have used my email address as the reply to address but when submit button is clicked it says 'unable to display page' can someone please offer any advice on how to set up this form

i probably sound very dumb, but any chance of a step by step guide on this as i am still very confused and what i tried based on what u said didnt work, i keep getting an error message 'please insert vcalid url address' i have tried verything i can think of and it didnt seem to work.

I once created a subweb on an FP site where I could only email to an address within the domain. Good security measure. So I got myself an email within that domain and had it then forwarded from there to my private email address. Check if the host has imposed this restriction. Although when I asked help they didn't know and I had to work that one out for myself.
