When chkdsk says that it has "found and fixed" an error just what is the fix?

It depends on what the problem was that was fixed. If it was cross linked files then chkdsk unlinks them and re-writes them. If it was incorrect file length reported then it recalculates the file length and rewrites the index information. If it was physical damage to the surface of the disk then it saves any recovered data to another part of the disk and flags the damaged area as unusable for future disk writes.
