Macros in MS Word

I want to set up a macro in Word. I have a template into which certain details must be entered. I want the macro to display dialogue boxes with text entry boxes on and OK buttons, so that the user will be prompted for the required information, be able to type it into a box and click OK for it to be inserted at the appropriate point. I have a vague recollection of an article in a magazine some time ago which showed how to do this but I can't remember.

Can anyone help me out?

I have had a look at the webpage that you suggested but it seems a little complicated! Can anyone tell me in plain English how to make these dialogue boxes? Is it possible without having to manually code?
